Patients with one or more of the following conditions may be qualified to receive home health services.

• New Diagnosis
• Paralysis
• Wound needing care
• New Medication teaching
• Medication monitoring
• Walking or movement impairment
• New oxygen regime
• Bedridden
• Just had surgery
• Joint replacement or repair
• Need infusion therapy
• Regular injections needed
• New medical equiptment (blood glucometer)
• Need PT/INR testing and monitoring
